How to Apply
- Go to the online application portal: http://forms.uhs.edu.pk/ugpadd
- Check your eligibility (If you have 60% or above marks in HSSC/equivalent exam, you can proceed with your application otherwise not).
- Fill in the application form with required information (personal details, qualifications, etc.).
- On initial submission, a password will be generated and it will sent to your e-mail address. You can now Log-in with the help of this password.
- Determine the Order of Preference for the programs as per your choice.
- Before final submission of your application, create a challan of an application processing fee of Rs.3500/ (Non-refundable) per program. You can submit the fee through this challan at the nearest branch of the Bank of Punjab (BOP) before the deadline of application submission. Paid challan slip must be uploaded on the online application portal.
- Upload the required document (Max.Size: 1 Mb/each document) as mentioned on the application portal.
- Verify the filled information and attachments.
- Make the final submission of your application form. After final submission, a confirmation notification will be received on your e-mail address. Please note that the application shall not be editable after final submission.
